Applying for a Travel Grant
The Eye on Grace travel grants are available only for people diagnosed with Ocular/Uveal Melanoma who must travel more than 25 miles for treatment. Eligible individuals are required to provide proof of diagnosis and proof of treatment-related travel.
How it works:
Submit a Travel Grant Application. (See below)
Watch for an email response from Eye on Grace with next steps.
Submit a Travel Reimbursement Form with receipts for each trip.
We award grants of up to $1,000 per person/per treatment to cover expenses for the patient and one caregiver. This includes lodging, travel (flight or train tickets/gas), Uber/Lyft, food, and parking.
A patient may receive a maximum of $10,000 per year with a maximum lifetime award of $25,000.
