The first step is to complete this Travel Grant Application. You must only do this ONCE to become part of Eye on Grace. When we receive your application, we will reply to you via email with next steps. In the future, when you want to be reimbursed for travel, you’ll use this Travel Reimbursement Form to submit your receipts.

Once you submit your Travel Grant Application, we will review it to ensure a diagnosis of Ocular Melanoma and corresponding treatment. Someone from our team will contact you via phone for a brief conversation to learn more about your OM journey. If everything is legitimate, you will qualify for a grant.

The Foundation exists because we believe in God and desire to show his mercy and love to Ocular Melanoma patients. However, that does not in any way influence how grants are distributed. We support all who come to us with a legitimate diagnosis, regardless of religion, orientation, etc.

Good news: no! Life with Ocular Melanoma is challenging enough without additional hoops to jump through. If you feel you need help with travel expenses, complete a Travel Grant Application. We will not ask for tax returns or require you to prove need. However, if you can comfortably cover your travel costs (or even a portion of them), keep that in mind while applying. We want our resources to stretch to benefit as many as possible.

Use this Travel Reimbursement Form to submit your travel expenses for payment each time you travel. It allows you to upload copies of your receipts, so make sure you have taken screenshots or photos of them. The form allows you to submit up to 15 receipts. If you have more than this, you should email us.

It helps with our record keeping if you will submit a Travel Reimbursement Form for each individual trip.

We respond very quickly, often the same day submissions are received, and always within two or three days (depending on holidays/our own treatment schedule/etc.).

Our preferred way to reimburse you is via Venmo or PayPal. If you don’t use these services, we can mail you a check. The Travel Reimbursement Form asks you to share your preferred payment method.

Yes, we can work with you if you are having financial trouble booking the tickets you need. Just reach out to us at info@eyeongracefoundation.org, and we can discuss.

Our bylaws very specifically state that Eye on Grace Foundation exists to help alleviate the burden of traveling for OM treatment. Therefore, we can cover only travel-related expenses. These include flight/train tickets, hotel, food, parking and gas. If you need help in other areas, we recommend checking out our Resources page.

YES, and that is one of the important things that sets us apart. We believe it’s important for OM patients to have the emotional and physical support of a caregiver when traveling for treatment, so we cover their travel-related expenses as well.

Yes, our bylaws state that a patient can receive a maximum of $1,000 per person/per trip. So a trip for you and a caregiver is capped at $2,000. We also have a $10,000 annual limit for both the patient and caregiver. There is a lifetime maximum limit of $25,000 per patient/per caregiver.

There are many ways to help! Check out our Sponsorship page for information about supporting the overall program or a specific event. You can also choose to make cash or stock donations. We have several major fundraising efforts throughout the year, most notably our Eye Patch Challenge in May, Giving Tuesday in November, and end-of-year giving. You can help us promote these on social media, form fundraising teams, host a Facebook Birthday fundraiser, give in honor or memory of loved ones, and more. Reach out if you have ideas!
